Fans of 22 Kids And Counting are in for a heartbreaking episode on Sunday night that sees Harley Radford break down in tears after a life-threatening diagnosis.
Harley is forced to stop working because of his failing health and this has a huge knock-on effect on his young family.
In a bid to keep their heads above the water, his wife, Millie, also 24, returns to work and he takes over her previous role of being a stay-at-home parent to their three children.
She then attempts to cheer Harley up with a puppy named Bella, before further tragedy strikes the family when a lump is discovered on the dog's stomach.
The young couple are then told that unless they are able to pay £2,500 for life-saving surgery, Bella will have to be put to sleep - forcing them into an impossible situation at a time when they are already financially stretched.
Millie says: 'The thought of losing Bella was a massive shock. I definitely wasn't expecting it. I was frightened to have to explain it to the kids, and obviously, Harley would be heartbroken, so that would be just destroying.'

Reflecting on how upset the possibility of losing the puppy made him, Harley says he felt emotionally 'drained'.
He admits: 'When [the vet] said Bella could die, I was really upset. I can't really explain; it's when you feel like something has just been drained out. You are just stressed and really worried.'
So, in a bid to save the family pet and further heartache for her young family, Millie and Harley start to sell their unwanted belongings to make some cash.
Millie then decides to pawn one of her prized possessions - her wedding ring.
Revealing the drastic action she has taken after the fact, Millie tells Harley: 'I've pawned my ring.'
He replies: 'You've pawned your ring. You’re joking me?'
After having it confirmed that it isn’t a joke, he adds: ‘I don’t understand. Her wedding ring is very important.’
It’s then revealed that even the sale of the ring isn’t enough to fully pay for Bella’s surgery, but viewers will have to tune in on Sunday to find out if the young couple are able to save her.

Viewers were left in tears as ‘struggling’ Josh Radford received a ‘heartbreaking’ diagnosis in a special episode dedicated to his health battle on Sunday.
Fans of the Channel 5 series, which follows parents Sue and Noel Radford and their 22 children, took to social media to open up about their ‘heartbreak’ over the scenes that saw Josh, 18, diagnosed with an anxiety disorder and agoraphobia.
In the special episode, titled Josh’s Story, which hit screens on Sunday (February 8), the teen was seen celebrating his 18th birthday with his family.
But it soon became clear to his parents Sue, 50, and Noel, 55, that something wasn’t right, as he batted off suggestions of learning to drive, and struggled to enjoy the party organised by his family.
Later scenes saw Josh refusing to get up from his bed, and explaining to his family that he was struggling with his eyesight.
While dad Noel at first assumed that his son was just being ‘lazy’, Josh’s behaviour continued to take worrying turns, and he was soon rushed to specialists for help before being diagnosed with anxiety and agoraphobia.
Complaining about the changes in his vision, and an increase in severe headaches which made Josh feel like he was ‘going blind’, Sue took the teen for an eye test.
While nothing was found to be seriously wrong with his eyes, Josh was then later rushed to the doctor - after suffering a panic attack in the early hours of the morning.
Visiting the GP, it was explained that Josh was experiencing the symptoms of anxiety - and was also diagnosed with agoraphobia. Often, those with agoraphobia avoid situations that cause anxiety and stress.
For Josh, this meant he spent weeks not leaving the house, with his anxiety becoming so bad his family noticed he was becoming increasingly quiet.
Opening up on his reasons for sharing his story, which was filmed over seven months, Josh said: 'I do hope that me doing this does give people courage to also open up and ask someone for help.'
The scenes left fans at home 'heartbroken', with one penning: 'I'm really sad about this I hope Josh gets the support he needs. I've bene crying since the beginning.'
Another chimed in: 'I just want to give Josh a hug, really feel for him,' as a third person said: 'Hope Josh is OK. One step at a time love.'
