The Heathcliff heartthrob had attended the UK premiere of the highly-anticipated romantic film in Leicester Square in London the night before.
Pictures show Jacob, 28, and Madi, 29, leaving in the early hours of Friday morning in the capital.
The pair walked out of the venue just seconds apart from each other as Jacob appeared to try his best not to be snapped with the model.
Jacob looked partied out as he wore black shades with his white shirt untucked from his suit trousers.
He and his female companion made a speedy exit once they got into the same chauffeur driven car.
It comes after Daily Mail revealed last month how the Australian actor had appeared to reunite with his on-and-off girlfriend Olivia Jade.
They sparked reunion rumors as they were seen leaving a hotel in New York together.
Snaps showed them climbing into an SUV together and were joined by Elordi's beloved dog, Layla.
Jacob had previously been spotted putting on a cozy display with Cuban beauty Ana de Armas, 37, at the Golden Globes in Beverly Hills.
He and Olivia's relationship began in 2021 - with the stars splitting last year in August.
However, they briefly rekindled their romance until officially going their separate ways in October.
'It's fully over. They are not getting back together,' a source told People at the time.
